Endoscopic gastric balloons and weight loss (bariatric) surgery have been shown to achieve greater weight reduction compared to any diet or medical therapy. Whilst it is not a replacement for lifestyle and dietary change, it greatly reduces your chance of weight regain in the long term. ENDOSCOPIC GASTRIC BALLOON: A temporary (up to one year) fluid filled sphere placed inside the stomach through a 20-minute endoscopic procedure performed under local anaesthetic throat spray or sedation. Gastric balloons delay food emptying from the stomach and reduce the space available. Weight loss is around 2-3 stones (approximately 30% excess weight lost). Some patients develop short-term side effects of reflux or nausea, though newer adjustable balloons are now available which can be deflated while they remain in place without needing to be removed to reduce these symptoms if they become persistent. Balloons must be removed by a repeat endoscopy at which point lifestyle changes will help keep your weight under control once it is no longer in place. LAPARASCOPIC SLEEVE GASTRECTOMY: Permanently removes 80% of the stomach reducing it to a carefully sized tube (“sleeve”) which creates a permanent feeling of fullness with smaller meals without need for any adjustment. The part of the stomach which is taken away produces hunger hormones so removing it also helps reduce appetite leading to excess weight loss of 65-70%. This operation does not involve the small intestine and can be a less risky option for those patients who desire an operation but have internal scar tissue from previous operations. LAPARASCOPIC GASTRIC BYPASS (ROUX-EN-Y): The stomach is divided creating a small pouch which is connected directly to your small intestine, missing out most of the stomach’s capacity and the digestive enzyme circuit. Downstream the small intestine carrying digestive enzymes is rejoined. This means you feel fuller more quickly and digest and absorb fewer calories from your food. On average patients lose about 70% of their excess weight or more with the gastric bypass. It can also improve the control of diabetes.
